Tax Exempt Forms

Please complete the applicable state tax exempt certificate if the following options are true for your business:

  • - If this is your first order with Apothecary Products
  • - The order is being delivered to one of the taxable states listed below
  • - The order should be tax exempt

 

The following states are taxable by Apothecary Products:

AL*, CA, CO, FL*, IA, IL, IN, KY, MA, MD, ME, MI, MN, MS, NE, NC, ND, NJ, NV, OK*, PA, RI, SC, SD, TN, TX, UT, VA, VT, WA*, WI, WY

*For orders shipping to Alabama, please provide your sales tax license.

*For orders shipping to Florida, please provide:

  • - A copy of your 2020 Florida Annual Resale Certificate issued by the FL Department of Revenue. Expired certificates cannot be accepted.
  • - A copy of your current Certificate of Exemption issued by the FL Department of Revenue.

*For orders shipping to Oklahoma, the resale and exemptions certificate is provided by your state. 

*For orders shipping to Washington, please send us a copy of your WA reseller’s Permit. If you are exempt from sales tax, please provide the completed exemption form.

 

The following states are currently non-taxable by Apothecary Products. However, we recommend submitting your sales tax info to us now so there are no order delays in the event that we collect sales tax from your state in the future.

AR, AZ, CT, GA, HI, ID, KS, LA, MO, NM, NY, OH, WV
